Thermage uses a monopolar RF method. As high-frequency current flows from a tip pressed against the skin surface to a grounding pad attached to the body, resistive heat is generated in the dermal tissue along that path. While the epidermis is protected by a dedicated cooling system, the thermal energy is delivered intensively to the deep layers of the dermis.

The generated heat denatures the triple-helix structure of the collagen fibers in the dermis, causing immediate contraction.
